When someone starts to get defensive, or I-centric, we can keep ourselves open and remain we-centric by utilizing co-regulation to help them shift their chemistry. By up-regulating we-centric behaviors like priming conversational space for trust and asking discovery questions, we can elevate engagement and levels of trust. A 2025 study1 examined for the first time the impact of someone’s conversational role as a speaker, addressee, or overhearer on their subsequent memory of the conversation.
A single-turn factuality test will miss much of this behavior because it does not examine how an error changes the trajectory of later turns. Final-answer scoring can also hide a conversation that failed repeatedly and recovered only after the user supplied corrections. EvolIF’s process-oriented evaluation was designed partly in response to this limitation in fixed, final-answer benchmarks. One of the most effective ways to ensure high reliability is through personalized, data-driven learning. By assessing the knowledge and judgment of healthcare professionals, organizations can identify variations in care and address them proactively. Through co-creating conversations that focus on how we can cooperatively tackle challenges, we activate an appreciative mindset, changing our neurochemistry.
The most common mistakes in unsatisfying conversational attempts are one member’s talking too much, getting too personal too quickly, and/or ignoring the fact that the other person is disinterested. Pursuing a one-sided conversation by adding too many details is likely to be boring to the listener. And not pausing to let the other person into the conversation is discourteous. The quiet member is often resentful about the speaker’s usurping the attentional space but is uncomfortable in saying so.
Design A Culture Of Excellence In Healthcare With The Baldrige Framework
A key difference is that validity refers to what’s being measured, while reliability refers to how consistently it’s being measured. For example, the Minnesota Multiphasic Personality Inventory (Hathaway & McKinley, 1943) has subscales measuring different behaviors, such as depression, schizophrenia, and social introversion. The split-half method would not be an appropriate way to assess reliability for this personality test. An alpha of .90 for a depression questionnaire, for example, means respondents’ scores correlate highly across the different symptom items.
Once people find giving feedback easy and realize it produces positive changes in how they interact with others, they will integrate it into their everyday life. Crucially, our study builds upon earlier work by Kim et al. (2026) to reveal that model scale does not resolve these failure modes. More recent works have explored sycophancy in healthcare settings. Chen et al. demonstrated that LLMs align with false medical information that users insist is accurate at rates up to 100% Chen et al. (2025b). Similarly, Rosen et al. showed that these sycophantic tendencies can exacerbate medical misinformation, where models prioritize pleasantness in user interactions, over correcting a user’s medical misconceptions Rosen et al. (2025).
Quantifying Conversational Reliability Of Large Language Models Under Multi-turn Interaction
In this post, we will explore what reliable communication means, different forms of communication, and essential guidelines to improve your communication skills. In summary, our study underscores the importance of safety-centric evaluations which investigate not only proficiency and domain knowledge, but efficacy over multi-turn conversations that more closely resemble real-world usage. Risk depends on the interaction among conversational depth, constraint density, evidence placement, revision frequency, task complexity, model version, memory design, and the consequence of error. A casual brainstorming chat may remain useful after dozens of turns because the cost of a missed preference is low and the user can correct the course. A five-turn conversation may already be unreliable if it contains conflicting legal authorities, several revised numerical assumptions, or an instruction whose failure could affect a person’s access to a public service.
Sijtsma (2009) later warned that alpha is widely misused as proof of a scale’s quality. A high alpha does not show that a scale measures only one thing. A set of items covering several different concepts can still produce an alpha above .90 if there are enough of them. Confidence that findings are replicable depends on ruling out individual rater bias. A typical assessment would involve giving participants the same test on two separate occasions.
NIST’s Generative Artificial Intelligence Profile states that generative-AI risks and long-term performance characteristics are less well understood than those of many non-generative systems. It advises organizations to consider different human–AI configurations and, where warranted, additional human review, tracking, documentation, auditing, change management, and management oversight. The profile is voluntary guidance, not a certification or a guarantee of compliance. The user must remember which instructions remain active, detect when a correction did not take, restate the controlling facts, and decide when the conversation is too contaminated to continue.
- But most organizations do not routinely collect data about high reliability principles.

Authors have final sign-off on their articles and complete statements that disclose potential conflicts. Founded in 2010, The Conversation is an independent, not-for-profit media outlet. Academics, edited by professional journalists author articles, and freely available online and for republication through a creative commons license. The Australian website launched in March 2011, and has expanded into editions in the United Kingdom (UK) in 2013, United States (U.S.) in 2014, Africa in 2015, France in 2015, Canada in 2017, Indonesia in 2017, Spain in 2018 and Europe and Brazil in the 2020s.
The user may introduce a source in turn three, replace it in turn nine, narrow the audience in turn fourteen, correct a date in turn eighteen, and request the final analysis in turn twenty-four. The response can read cleanly while relying on the discarded source, the original audience, or the superseded date. The conversation simply stops preserving its own state accurately. To understand the causes of reliability degradation, we analyze results by conversation length, task complexity, and entity extraction scenario type (see Appendix for full tables).
This behavior has been attributed in part to reinforcement learning from human feedback (RLHF), where, in optimizing models toward helpfulness, they are inadvertently taught to prioritize this helpfulness over factual accuracy. The LoCoMo benchmark examined very long conversational histories spanning hundreds of turns and multiple sessions. Its authors reported that long-context models and retrieval-augmented approaches improved results, even though they continued to struggle with long-range temporal and causal relationships and remained substantially below human performance in the study. The journey toward high reliability in healthcare gained momentum more than two decades ago with the release of the Institute of Medicine’s (IOM) landmark report, To Err is Human (1999), which revealed shocking statistics about preventable deaths in hospitals. This pivotal report was a turning point and catalyst for healthcare organizations to move beyond complacency and take definitive action to reduce harm and improve patient outcomes.
Here, scaling parameter counts from 8B to 32B offers no consistent improvement from this baseline compliance. To understand how this conversational structure influences reliability, we first establish a baseline for accuracy on isolated binary decisions, representing an LLM’s reliability when weighing two arbitrary hypotheses. As expected, narrowing a larger answer-space to a binary choice between the correct answer and a single distractor yields consistent improvements in accuracy across all models and datasets. Averaged across models, reducing the scope of answer options improved accuracy by 33%, 26%, and 26% on MedQA, MedMCQA, and JAMA CC, respectively (Figure 2).
Conditioned on the model successfully selecting the target answer in the previous turn, we introduce a new alternative hypothesis (distractor) from the original answer space. The model is prompted to either stick to its previous selection or switch to the newly presented option. All evaluations utilize few-shot exemplars drawn from the respective development sets of each dataset (MedQA, MedMCQA, or JAMA CC). The test conversation should then change that state deliberately.
Writing an email is communication; writing in a diary is expression. In this session we’ll be taking participants through a “self-assessment” of their current maintenance practices. What we’ve observed is that maintenance typically directly controls only about 10% of most losses from ideal production, and at most 30%. That said, excellence in Lauradate reviews from users maintenance practices is essential for a reliable plant. In this session we’ll be taking participants through a “self-assessment” of their current stores and purchasing practices.
